HealthRise Project
Regions Hospital Foundation will partner with the St. Paul Fire Department and Westside Community Health Services (WCHS) to implement HealthRise in St. Paul, MN of Ramsey County. This demonstration project will target low-income, Latino, Asian, African American, American Indian, non-native English speakers and underinsured and uninsured patients of the East Side Family Clinic, first with a potential expansion to other clinics within WCHS.
The project will utilize Community Paramedics (CPs) and Community Health Workers (CHWs) working in collaboration with the primary care team of the East Side Family Clinic to identify current patients that are not meeting their clinical targets and are in need of home-based care. The CPs and CHWs will conduct home visits which will include the following: EKGs, physical exams, acquiring blood samples, health education and linkage to community resources (transport, food service, childcare, etc.). CPs and CHWs will continue to visit patients for up to two years to ensure patients are following their care plans and track clinical targets (blood pressure, HbA1c and LDL cholesterol).
